Participation, Acknowledgement, & Assumption of Risks

Please take a few moments to read and understand this legal document before you sign. Minors must have an agreement signed by a parent or court-appointed legal guardian.

for minors:

  1. One parent must create a login for themselves FIRST.

  2. While logged in, navigate to your Profile (look for the profile icon) then Household.

  3. Click the Add a Household Member button and complete the form.

  4. Navigate to Sign a Waiver and select all household members for whom you’d like to complete a waiver (all adults must complete their own).

  5. Complete the form, then you’re all set!
